Wheldrake

Due to health and safety reasons our Wheldrake branch remains closed at this time.  Prescriptions can be collected on a Friday between 2-4pm from the cottage in the grounds of Wheldrake church.

Patient Information

Medical records

Your medical record is yours, and you have the right to view it at any time. It can be useful to have this information to hand when you are coming to see your doctor, or if you are due to attend an appointment at hospital or with a specialist. Your medical record contains the following information:

  • Appointments – dates, who you saw, reason for visit, history, examination, outcome
  • Medical diagnoses, investigations, procedures
  • Allergies
  • Medications
  • Vaccinations
  • Test results
  • Links to information about your diagnoses/treatments

You can view your medical record online, to help you look after your own health. To do so you must make a request through our Patient Access service, which will be approved by a doctor at the Practice. This is a secure service to which only you will have access.

We have a duty of care to ensure that our patients are able to understand and interpret the information contained in their medical record. As such we reserve the right to assess every patient’s request to view their record and limit access if we believe that serious harm could result. This may be done in consultation with external bodies such as mental health organisations.

We do not provide medical record access for any patients under the age of 16.
