Patient Information
Carers are people who, without payment, provide help and support to a family member, friend or neighbour who cannot manage on their own due to physical or mental illness, disability, substance misuse or frailty brought on by old age.
Anyone can become a carer; carers come from all walks of life, all cultures and can be of any age including children. Many feel they are doing what anyone else would in the same situation; looking after their mother, son, or best friend and just getting on with it.
Carers don’t choose to become carers: it just happens and they have to get on with it; if they did not do it, who would and what would happen to the person they care for?
If this sounds like you, you are probably a carer. Support is available to you.
If you feel you are a carer you should let the Practice know. We can offer you annual health checks and make arrangements to make our doctors as accessible as possible for you.
Please complete the Carer’s Identification Form found here or available from Reception.
