Our Services
Having a single annual review is the quickest and most convenient way of monitoring long-term conditions. Without regular check-ups, conditions can worsen and complications might arise that damage your health. Proactive management of long-term conditions can alleviate symptoms and let you lead as happy and healthy a life as possible.
To ensure that patients with long-term conditions only have to attend the Practice when necessary, we now invite these patients to attend the Practice for a single annual review in the month of their birth.
During one appointment, a thorough review of all recorded conditions will be undertaken. Depending on the individual case this may require initial blood and urine tests followed by a face-to-face review with one of our practice nursing team. In some circumstances a further face-to-face chat with a GP may also be necessary.
You will receive a written invitation in advance of the planned review to allow time to book the necessary appointments. The letter should include a blood test form if one is required, please bring this with you to your appointment.
