You can request a repeat prescription by:-
- Post using the right hand portion of the computer generated prescription
- Telephone 01904 724882 Please provide these details when ordering
- Fax 01904 608710 using this form
Please give as much notice as possible when ordering as we need a minimum of two working days notice to process and dispense your prescription request. Some items will need to be ordered from our supplier which adds delay to the processing of your prescription.
Please note that children under 16 cannot collect prescriptions.
At intervals your medication will need to be reviewed and you may be asked to make an appointment to discuss this.
