Annual birth month review explained…
As many of you will realise the practice has recently introduced a new scheme in an effort to make annual “check-ups” for any patient with long-term conditions more simple and straightforward for all concerned. We were conscious that some patients were understandably frustrated at repeated invitations for “review”. Our aim is to complete a thorough review in AN INDIVIDUAL patients’ birth month. Depending on the individual case this may require initial blood and urine tests followed by a face-to-face review with one of our practice nursing team. In some circumstances further face-to-face chat with a GP may also be necessary.
We anticipate that written invitations will be sent in the month before the planned review to allow time to book the necessary appointments. The letter should include a blood test form if that is necessary.
A range of conditions are covered by this scheme including diabetes, heart disease, stroke, high blood pressure, atrial fibrillation, kidney disease, asthma, COPD, thyroid problems, rheumatoid arthritis and epilepsy.
We sincerely hope that this will allow us to ensure that we provide an up to date and effective review , detecting problems or complications at an early stage, while minimising disruption to all our patients.
We apologise for some of the “teething” problems that may have arisen during the initial launch of this scheme and hope that all our patients find this to be a refreshing change.

Patient Partner enables patients to book, check or cancel appointments 24hrs/day 365 days/year via their telephone hand-set. All that you need to use the system is your date of birth and your telephone number.
When you ring the surgery you will be presented with menu options to help direct you correctly. Please listen carefully to the available options.
Press 1 for the automated booking system
Press 2 if you have a medical emergency
Press 3 to request a repeat prescription or leave a message for the dispensing team
Press 4 to speak to a receptionist
Press 5 to speak to the practice secretary who is available 9am to 12 noon Monday to Friday
